New Patient Registration
Security of your medical record is important to us, and you will therefore need to bring:
- Proof of identity (e.g. passport, photo card driving licence etc).
- Your NHS medical card. If you do not have this we can still process your registration but it may take longer to trace your medical records and you will need to fill in more paperwork.
- Proof of address (a utility bill less than 3 months old and not for a mobile telephone).
- Records of immunisations – we need the red book for all children registering with the practice
You will need to do this both for yourself, and any other members of your family who also want to register. When we receive your details we will complete the paperwork for you and contact you to get your signature, to arrange registration and co-ordinate your new patient check.
As part of the registration process you will also be required to complete a health questionnaire, and to attend the practice for a New Patient Check. Please bring any medication with you that you are taking.
