Welcome to Brentfield Medical Centre

The Brentfield Medical Centre offers care to a multicultural community including some of London’s most deprived electoral wards. We are a multicultural team whose ethos is to serve our patients with the best medical care that we can with dignity and compassion. We believe strongly in education. We are a teaching practice training GP Registrars (GP trainees), medical students and student nurses.

Our vision is to offer our patients a wide range of high-quality services targeted to best meet their needs. We hope this leaflet will offer you all the information needed to access the services you require from us at Brentfield Medical Centre.

Online Consultations


Please use the link below to complete an Online Consultation Form:

You can now contact your GP online and get a reply within one working day from either the Practice Team or our Harness Clinical Team of GP's.

A member of the team will review your consultation form and reply to you via email or telephone in *one working day, with advice or an appointment.

*forms submitted at weekends will receive a reply at the latest by the following Tuesday.

Register for Patient Access 

Patients aged 16 and above are now able to make or cancel their appointments on line, to order their repeat medication and to view a summary of their medical record.   To find out more, please read the Patient Online Leaflet.pdf and Patient Online Frequently asked questions.pdf    

If you wish to use these services please complete the Patient Online Registration Form.docx and bring it to the Practice.  You will need to provide ID to confirm you are the patient in question and we will issue you with an unique ID number and the details of how you can register for this service.  Alternatively, you can speak to a receptionist, or if you have any further questions email the Practice at or write to the Practice. 

We hope that this helps patients to access our services and manage their care more effectively at a time that is convenient to them.  It should also reduce the number of calls to the Practice and hopefully reduce delays on the telephones. 

Brent Health App 

Brent Health App

A new free app providing up-to-date information about health services is now available in Brent in response to calls from patient groups. Brent Health App’s purpose is threefold:

    1. To communicate health news
    2. Help find local services
    3. Provide healthcare information.

Users can also book GP appointments, order repeat prescriptions, access summary medical records, find local services and use as a symptom checker.

To add the app to your phone for free, search Brent Health App in Apple App Store or Google Play.


Friends and Family Test

If you would recommend the care you receive at the practice please complete the Friends and Family Test by logging on to you can also complete a questionnaire in the practice.    We are also very happy to receive feedback from patients and their families to help us continually improve the care of our patients.

7 Day Access To GPs

The practice is working with the other local practices in Harness to provide additional GP appointments between 6-9pm Monday to Friday and 9am to 3pm Saturday and Sundays.  If you would like an appointment with the hub please speak to a receptionist who will be happy to help you.

Opt out form

The practice are participating in the North West London whole system integration programme.    This includes modelling the new model of care using anonymised data extracted from the practice clinical data base.  The same process is happening with hospital, community and social care information to help provide more joined up care to patients. The leaflets above provide information on the data extraction process and an opt out form for patients who do not wish their medical record to be included in the process.    The practice take their responsibility for the protection of patient information very seriously and ensure we have robust systems to maintain patient confidentiality at all times.   If you would like to discuss this further please email the practice on
